If you have moderate NAT its generally fixed by port forwarding. You will need to give your Windows PC a fixed IP address, find out what ports Black Ops 2 Zombies uses, and then forward those ports from the 5366 to the IP address of the Windows PC.

PS4 shows NAT-Type 2, Black Ops 4 and WWII did show Moderate NAT type. My PS4 is cable, into a router that is cable from internet provider. I did go into internet providers router and port forwarding port to my second router. On my second router I did port forward ports to my PS4.
